Facelift
or Facial Filler:
The basic
guidelines are: A good candidate for a facelift will have
jowls, heavy nasolabial folds or a sagging neck. Facial filler
candidates will have moderate facial volume loss, weakness
in the cheeks, and moderate nasolabial folds with moderate
jawline contour changes.
In today’s
world where we all want maximum improvement with minimal down
time there are multiple choices.
The choices include fillers and surgical procedures.
FILLERS:
- Semi-permanent, permanent and short acting.
- Short action 6-12 month

FACELIFTS:
There
are basically two types of face lifts.
- Minimal
access-short scar
- Traditional
Minimal
Access Face Lift:
There
are multiple names for this procedure with only slight variations.
- Minimal
access or MAC
- S lift
- Quick
lift
- Life
lift
- Temporal
lift
Procedure:
An incision starts just under the temporal hair line in to
the ear and out to stop at base of ear lobe. The skin lifted
is a moderate amount. Area under is minimal.
Advantages:
- Less
invasive
- Reduced
recovery time, 7-14 days
- Reduced
undermining
- Less
bruising
- Can
be done with sedation and local anesthesia
Disadvantages:
- Cannot
get optimal neck lift; especially in a heavy neck.
- Sometimes
requires additional (scalp) brow incision to smooth temple
area
- Duration
of results is 4-6 years

Full or
Traditional Face Lift:
This procedure
improves cheek, jowl and neck.
The incisions
are made under temporal hair in and around ear extending into
posterior hair and an incision under the chin.
Advantages:
- Better
ability to shape cheek, jowl and neck
- Longer
duration, 8-12 years or almost double MAC lift duration
Disadvantages:
- Down
time, 2-3 weeks
- Risk
of complication slightly higher for bleeding and skin
problems than in Mac lift

NECK
LIFTS:
There
are two types, the full neck lift and the suture suspension
lift.
Suture
Suspension Neck Lift:
If there
is not a large excess of skin and fat the neck can be significantly
improved with suture support and liposuction if needed.
An incision
is made under the chin and fat is removed if needed. The muscles
under the chin are tightened and two enter-locking sutures
are passed to the area just behind and below the ear. They
are tightened to support the neck and redistribute the skin.
Advantage:
Usually minimal down time
Disadvantage:
Will not connect a heavy neck

Full Neck
Lift:
This is
a lower half of face lift. There is an incision under the
chin and behind the ear and in to the hair line.
Advantages:
- Smooth
neck better than with suture suspension, especially in
heavy neck.
- Reduced
down time
Disadvantage:
Cannot be done if jowls and cheeks are heavy
